What is Gynecomastia?

Causes of gynecomastia
Most cases of male breasts start in puberty as a result of sex hormone imbalance. Some of the adolescent Gynecomastia resolves without any treatment within two years. In cases where it does not resolve, the persistent breast tissue and fat deposits together cause psychologically disturbing male breasts. Gynecomastia can also occur as a side effect of certain medications, following steroid therapy, hormonal imbalance, and liver disease. Male breasts occurring in obesity as a result of pure fat accumulation is called ?pseudo Gynecomastia?.
What does it consist of?
Most cases of male breasts consist of a combination of breast tissue (glandular tissue) and fat. The proportion of these two elements varies. In body builders and men without much fat, it is mainly glandular tissue (puffy nipples). Some gynecomastias are predominantly fatty in nature, especially in obese men. The male breasts can enlarge as the fat deposition in the body increases and for this reason, male breasts are more common in adults.
When can you have gynecomastia surgery?
Surgery is sought typically by boys in their late teens and men between 20-50 years of age.
Is gynecomastia harmful?
Gynecomastia is not physically harmful. The presence of the abnormal enlargement of the breasts is psychologically disturbing for most men. More over it can restrict certain physical activities where you are expected to be bare chest as in swimming or certain religious functions in India. The glandular tissue will not decrease in size with exercises. So if you are concerned about the male breasts, surgery is usually the only solution. Keyhole surgery for gynecomastia
Gynecomastia can be reduced by minimal access surgery in most cases using special instruments and modified technique of Liposuction. Excess fat on chest is removed by liposuction. Remaining tough gland tissue is then removed through tiny incision under areola. This results in adequate correction of chest shape. The scars are tiny and not easily visible in most cases. Recovery after gynecomastia surgery
The surgery is performed as a day care procedure and you will be able to go back on the same day. A custom made corset will be put on immediately after surgery and should be continued for 6-12 weeks. Minor discomfort and soreness can be expected in the initial few days. Swelling in the operated area will be greatest in 48-72 hours and then will keep on reducing till 3-4 weeks after the surgery.
Numbness/ altered sensation on the chest can occur during the healing phase. You may resume light physical activities within 24 hours. Gym and other heavy workout should be withheld for at least a month after surgery.
Does gynecomastia re occur?
The glandular tissue of male breasts does not form again and thus the surgery provides permanent removal of the breast tissue. However, if you put on a lot of weight, fat cells on the chest can enlarge and can cause fullness of the chest again (pseudo Gynecomastia)

How to plan your keyhole male breast surgery-
Initial planning can be done through emails (drharimenon@gmail.com). If you send photos of chest in front and side views along with brief medical history, the grade of gynecomastia and approximate cost can be known. The date for first consultation and surgery can also be booked online or by phone.
Pre surgery preparation-
Gynecomastia surgery can be scheduled 3-4 days after the first consultation. Pre op preparations include blood tests, anesthesia evaluation and getting measured for custom made elastic vest. Heavy smokers should stop smoking 2 weeks before surgery.
On day of surgery-
Minimal access gynecomastia surgery is a day care procedure. You can return home/ to hotel in the evening. Pain medications will be prescribed. Normal diet, driving and routine activities can be resumed from next morning. You may proceed to a resort or some sight seeing after 24 hours.
Post op visit-
First post op visit is 3-4 days after surgery. Further frequent check ups are not normally needed. Second assessment is done one month after surgery if you are in Ernakulam then. Otherwise you may keep in touch with Dr. Hari by phone or e mail.
