Assessment/ planning of Rhinoplasty

Rhinoplasty is a surgery that requires detailed evaluation, discussion and meticulous planning. For an experienced surgeon, this will be effortless and second nature. The most important aspect in planning the surgery is to find out the needs and expectation of the person. Having understood this, the cosmetic surgeon will be able to tell you about what is possible and what is not. If the expectation is beyond surgical possibility, Dr. Menon will inform you as to what best can be done. A crucial component in planning in Rhinoplasty is assessment of the remaining facial features. The beauty of a nose is not appreciated in isolation, it should look  attractive on the face and for this reason, asessment of other facial features is important .The ultimate goal is to achieve facial balance and symmetry. Pictures of the face will be taken in different views and studied to evaluate the subtle aspects of nose. A thorough discussion will enlighten you regarding what are the problems and how well they can be addressed. Line diagrams on the pictures will create an approximate image of the proposed changes. (Dr. Hari Menon does not use software morphing to show expected results. This is in keeping with the ethical standards adopted by experienced Rhinoplasty surgeon?s world wide)

A psychological evaluation may be asked for in some cases to rule out body dysmorphic disorder.

This is done most often using implants (silicon/ Gore-Tex/ Medpore/ Biopore) without any cut on the skin. Cheek augmentation, facial fat reduction, lip plumping and dimple creation are other cosmetic surgery procedures that may be combined.  In open Rhinoplasty, there will be a small cut in the base of nose.  Once healing is complete this mark is not visible as it is in the shadow area of nose. There are scars in closed Rhinoplasty.

A complete discussion with your treating surgeon is most important. Smoking should be stopped 2 weeks before surgery. Blood tests and X rays/scans may be required few days before surgery. You may not be able to socialize actively during the initial few days after surgery, so make provisions for your social inactivity. 2 weeks time off from work is advisable.

The expected outcome of surgery should be discussed well with your cosmetic surgeon. Results of previous surgeries will provide an indication of what is possible. Line diagrams super imposed on photographs are also helpful. The results created by computer software may not be accurately duplicated in surgery. For this reason, computerized imaging is not used in most ethical rhinoplasty  centres.

Most Rhinoplasties involve modification of the shape of the nose bridge. In such cases, it is not advisable to wear heavy glasses till the swelling subsides. In cases where only the nasal tip of base of nose is altered. glasses can be worn.

In open Rhinoplasty, the change in shape to cartilages and bony structures are effected under direct vision. Open rhinoplasty involves tiny incision in the columella (the partition between nostrils). The mark of this incision fades in time and is hardly visible. In close Rhinoplasty . all incisions are made inside the nose without any external marks.

A proper Rhinoplasty/ cosmetic nose surgery does not hamper the functions of nose. Breathing can be improved by Septo Rhinoplasty. Sense of smell is not affected by Rhinoplasty.

Cosmetic Rhinoplasty in Kerala is usually performed in the age group 18-45 years. In younger ages, the facial development is not complete. Rhinoplasty can be performed in older age groups, to correct ageing changes, revision rhinoplasty and post traumatic deformities, though healing may be affected by concomitant medical problems and other factors.

Rhinoplasty involves change in external nose architecture. Major complications are rare in Rhinoplasty . Dissatisfaction with the outcome is possible. infection, complication from anesthesia, asymmetry, implant mal position, nose bleed, pigmentation and allergy to medicines are possible. The risk of complications can be reduced by selecting a qualified and experienced surgeon performing Rhinoplasty in quality accredited centre, conveying your wishes clearly to the surgeon and by following all post operative instructions.

Rhinoplasty can be combined with other cosmetic procedures if needed. Most common cosmetic procedure performed with Rhinoplasty is Chin augmentation. Other facial cosmetic surgery or body contouring surgery can also be combined with Rhinoplasty.

Short bulbous nose is a common cosmetic problem in Kerala. Rhinoplasty can create significant improvement and make the nose well defined. The cartilages that give shape to the tip can be altered to make the tip more pointed. most cases need addition of graft or implant to the bridge of nose to make it look longer. The base of the nose can also be narrowed to create overall improvement.

Wide nose can be corrected by cosmetic nose surgery/ Rhinoplasty. The increased width may be due to wide base of the bones in upper part of nose or wide base of nose or combination of both. The bones can be narrowed and the base can be reduced to the right proportion. Wide nose correction is commonly performed in Kerala.

Long nose can be downsized to match the rest of the face. The facial balance will be assessed and if an additional procedure like chin augmentation is required, that will add to the overall improvement.

Hump on nose can be corrected by Rhinoplasty. The excess bone and cartilage in the hump can be filed down or may require excision. The nasal tip also needs to be modified to match the newly set profile of nose. The drooping tip can also be completely corrected by changing the orientation of the cartilages on the tip of nose.

The first step is to contact your operating surgeon and inform him/her about your state of mind. The final result of  Rhinoplasty takes many months to define. In the initial few weeks, there will be some swelling which may mask the actual work done. It will be prudent to wait for at least 3-6 months before considering revision Rhinoplasty . Some situations may warrant immediate correction- mal positioned graft/ implant, haematoma, infection etc.

It is normal to have swelling on the nose after a Rhinoplasty. The swelling will be more in the tip area especially after open Rhinoplasty. Most of the swelling subsides in 2-3 weeks. In most cases the nose will look good in 3 weeks. Complete resolution of edema takes 6-12 months.

Rhinoplasty aims to create a better looking nose that matches the rest of your face. Any significant problems in facial balance should also be addressed at the same time for optimal results. Rhinoplasty will not make you look like somebody else, rather your face will be enhanced aesthetically retraining the essential character.
